OPT-OUT & SECURITY FREEZE INFORMATION

Introduction & Overview

ChoicePoint (NYSE: CPS) provides businesses, government agencies and non-profit organizations with technology, software, information and marketing services to help manage economic risks as well as identify business opportunities. Consumers have free access to the reports we create at www.choicetrust.com.

We have adopted policies and procedures to foster our commitment to privacy and our vision that good privacy is good business, including policies and procedures related to consumer choice. Consumers may opt-out of the dissemination of Personally Identifiable Information from ChoicePoint-owned marketing services databases (see below). Also, individuals may opt-out of ChoicePoint public facing products1 in accordance with laws or if permitted by ChoicePoint policy (see below). In addition, consumers in certain states or the District of Columbia may have the ability to place a security freeze on specific ChoicePoint consumer reports (see below).

ChoicePoint-Owned Direct Marketing Services Databases

We provide direct marketing services to customers to help their marketing campaigns reach consumers who are more likely to be interested in receiving such information. We routinely remove consumers from these services upon request.

To opt-out of ChoicePoint’s Direct Marketing Services Databases, please complete the form below. Your request will prevent your name and address from being used in unsolicited communications from ChoicePoint Precision Marketing and its customers. However, please note ChoicePoint is not the only direct marketing company in the industry and opting-out of our databases will not prevent other companies from sending you direct marketing materials or making telemarketing calls.

The Direct Marketing Association (DMA) (www.the-dma.org) also provides individuals with a form to use to have their names removed from other direct marketers’ mail, e-mail, and telephone call lists. ChoicePoint honors opt-out requests made through the DMA’s services.

We also comply with national Do-Not-Call (DNC) requirements and honor opt-out requests through that service. To add your name to the national DNC list, you may register at www.donotcall.gov. Many states have also created DNC lists for their residents.

Finally, we obtain and honor opt-out requests made through the three national consumer reporting agencies.

ChoicePoint Public Facing Products [1]

Individuals may opt-out of ChoicePoint public facing products1 in accordance with laws or if permitted by ChoicePoint policy.

Certain state laws provide that their law enforcement officers and specific public and elected officials can opt-out of ChoicePoint public facing products.1 In addition, public and elected officials, including law enforcement officers, may request to opt-out of ChoicePoint public facing products1 in cases where the official is on a high-profile assignment, or under threat of death or serious bodily harm.  Law enforcement officers and public and elected officials may submit their opt-out requests using the form at the link below.

Private individuals who are facing a substantial risk of physical harm or who are victims of identity theft may request to opt-out of ChoicePoint public facing products.1 Individuals who may request to opt-out for these reasons may submit their requests online or by mail. Each request must include documentation substantiating the risk of physical harm or the individual's status as an identity theft victim. Acceptable documentation includes a properly filed police report, a letter from a law enforcement agency, or the name and contact information of a law enforcement official who is familiar with the situation and able to provide confirmation. Requests may be submitted using the form at the link below. If the supporting documentation cannot be uploaded, individuals may submit their requests online, but also must mail the supporting documents to this address:

ChoicePoint Security Freezes
If you live in certain states or the District of Columbia, you may have the ability to place a “security freeze” on specific ChoicePoint consumer reports and subsequently release or "lift" a freeze at some later date.
